Please read the following procedure carefully when registering a Fond du Lac Phoenix Select Player for a Phoenix Team for the 2011-2012 season.
- Players will be notified of their status by the team coach by email or by phone after the last tryout for a team.
- When a player accepts a position on a team and notifies their coach, you may then begin your 2011-2012 Phoenix Player Registration.

- Login by using your existing NGIN Account or create one
- Existing NGIN Account
- Previous FDLSA players who registered online can use their existing NGIN Account and login using current username and password
- Creating a new NGIN Account
- If you're new to FDLSA or do not have a NGIN Account from another organization, you will have to create a NGIN Account to begin online registration
- After you fill in required information and click Sign Up For Your NGIN Account, you will receive an email from NGIN to finish activating your account
- Click on the activation link in the email to finish creating and activating your account. If you do not click the activation link in the email, you will not be able to continue to register

- When you are completing the online registration, please make sure that you pick the correct age-level and team color (Red, White, Black). Your coach or team manager has this information.
- During online registration, you will be asked to choose a form of payment. Payments can be made with your Visa, Mastercard, Discovery, or PayPal Account.
- There are three payment options to choose from:
- Option 1-Payment in Full at the time of registration
- Option 2-Payment in Two Installments. First installment due at the time of registration and the second installment due Sept 10, 2011 and will be automatically withdrawn
- Option 3-Payment in Four Installments. First installment due at the time of registration, second installment due Sept 10, 2011, third installment due Oct 10, 2011, and final installment due Nov 10, 2011 and will be automatically withdrawn
- After registering online, all additional forms should be printed and filled in. Forms are also listed under Registration Forms on the Select Soccer Page
- All forms are due at the scheduled team meeting date. Team meetings are set by the coach/team manager.
- All players should email a headshot picture for their player passes to their coach/team manager. Pictures may be shot at the team meeting. Check with the team manager/coach on which way to collect the players pictures.
- All new Fond du Lac Phoenix players will have to order a uniform. Ordering information will be provided by your team manager. Existing players who would like additional items will also be notified by their manager how and when to order. Click here for Uniform & Apparel page.
