Fond du Lac Soccer Association

When a player has received an offer to play for a Fond du Lac Phoenix team and the offer has been accepted, the form forms listed below has to be printed, completed and turned in. The form is available during the registration process and accessable here if you missed it while registering.  Please register on-line first and select your form of payment (credit/debit card in full or debit/credit card payment plan). Bring the form listed below to the team meeting. Team meeting dates will be provided once a date is set by the team manager/coach. 

If you are absolutely unable to attend the team meeting, please make arrangements with the team manager/coach to have the form delivered, before the team meeting date.

Remember, your player registration is not fully completed until you have:
  1. Completed the Online Registration and 
  2. Turned in the completed form below

Please print, fill out, and hand in the form below to the team manager/coach. Your child will not be allowed to participate if the form (marked with an *)  is not turned in.
